Shared Living Coordinator

Job Summary:
Oversees all aspects of new service recipient,  shared living/host home provider, Group Home and other residential placements and respite recruitment; provides interim program management duties for new service recipients; coordinates temporary respite services; and marketing Bethesda Lutheran Communities' services.

 Essential Job Functions:

  • Responsible for the recruitment and interviewing of Host Home providers by using a variety of recruitment methods, screens applicants, conducts background screenings, and completes application process.
  • Completes home inspection and HUD survey at each home.
  • Gathers and relays additional information from the Community Centered Board, Community Development Disability Organization and/or Managed Care Organization, other service agencies, family/guardians, etc. to the Program Manager.
  • Meets with the individual and participants of their choosing to determine needs and personal goals
  • Assists host home providers and program managers in locating temporary relief services (Respite)
  • Provides residential program management duties for all new service recipients for the first thirty to ninety days or for the frequency needed.
  • Completes necessary assessments including but not limited to, Health and Safety, skills assessments, and Comprehensive Life Review and completes inventory of personal possessions
  • Provides training and support to the host home
  • Puts together the initial master, home, and/or medical files
  • Maintains a data base for referrals and placements
  • Other duties as assigned

Supervisory Responsibilities (if applicable): 

  • During the placement period and 30 days after placement this position is responsible for establishing and implementing program goals, arranging or providing orientation and training, providing direct oversight of providers, maintaining a quality assurance program, developing and maintaining individual budgets, providing an environment that is conducive to the welfare of the service recipients and is in accordance with federal, state, local and organizational regulations.    ​​

Minimum Job Qualifications:

  • Education and/or Experience:
    Bachelor’s Degree in human service or related field and two years experience working with individuals with developmental disabilities.
  • Certificates, Licenses or Registration:
    Must have a valid driver's license and a clear driving record; must have cleared and approved criminal record check; must have legal status in the U.S.; must hold First Aid/CPR certification. Additional certifications may be required by regulations. 
  • Physical Demands:
    Must have ability to travel among assigned sites; must be able to assist with a two-person lift of an adult; must be able to do frequent bending and lifting; and must be able to stand for eight (8) hours at a time.
